easy-rsa with subjectAltName support (update)

This is an update to Vicente Aguilar's original article - I've updated the diff and package from easy-rsa 2.0rc1 to easy-rsa 2.0, and made a couple of other minor changes.

Changes:

• Smaller patch file (achieved by adding a new README, rather than renaming and replacing the existing README)
• Added the openssl.cnf modifications to openssl-0.9.6.cnf
• Modified pkitool so that it still generates usage when called without arguments
• Added samples of optional key parameters to the vars configuration file
• Fixed revoke-full script to work with modified openssl.cnf file

Note: easy-rsa 2.0 is part of the OpenVPN distribution. I've based my patch on the easy-rsa 2.0 part of the OpenVPN source tree.
